I have twin baby boys 3months 15days. They have been having gas problem. They are on formula and my feed both. After every feed they vomit some amount of milk in the form of curds. Even if they have burped they vomit.Is this normal?
How long will this continue? What is the reason and what can i do to help.

Hetal replied. at first,make sure that it is not vomit and is just spitting up.
since you have mentioned that they vomit some amt of milk,,its not vomit and is spitting which is perfectly normal..this curd like spitting is quite normal..its mostly because babies tend to suck more of air alongwith formula or BM.
please note that spitting is different from gastric or acid reflux.spitting is caused when baby takes air along with bm or formula,,air gets trapped betwn liquid and when it comes up,,alongwith liquid also comes up..this happens because a newborn stomach muscles are not mature enough to handle the outside nutrition..as until now he ws getting nutrition in the womb directly thru blood and it takes around 6months of time to get rid of this,,esp once the baby starts to sit,,this reduces or goes off !
even though this is quite normal and burping is expected to reduce this spitting up,,if it aggrevates thn your baby is suffering from what we call as gastric reflux or acid reflux ! further might worsen to GERD, if it prolongs for more tha 6months.
the severity of spitting up and that your baby is having gastric reflux is decided on the amt of fluid and whether your baby is vomitting or spitting up..if he just spits up which is just a gentle thrust,a couple teaspoons or let me say 1 oz or so,,its ok to spit up,,otherrwise if he is vomitting (higher thrust), which will also frighten your baby as well and he will cry after that,,it means he is havin reflux and not spitting up ! reflux happens because of malfunction of muscle between esophagus and stomach,,and due to this, stomach contents(curd like,undigested formula or bm with stomach acids),,comes to throat...and if this continues longer your baby may be prone to GERD and is likely to have ear infections in future,,all the later part of story..,,so you judge it well whats going on with your baby right now !
you can reduce gentle spitting and to some extent vomitting by :
1. hold your baby in upright position always and feed,,either while breastfeeding or formula.
2.after your baby feeds or completes the feeds,,dont allow him to get overstimulated,,let him calm and relax,,burp him every 2-3oz of formula or when you switch sides while breastfeeding.
3.make sure the hole of the nipple of your bottle is of the correct flow,,in acc to his age,,or has some control flow system or valve,,,and not too big or small that makes your baby frurstrated and may lead to gulping the formula..all this makes your baby suck in air more and formula or bm less.
4.after feeding or during feeding sessions, avoid ne pressure on your baby tummy,,let the diaper be loose,or his clothes are not tight around the waist line...
5.keep your baby in upright position even after feeds atleast for 15mins,,let him settle down all the formula he has taken inside and if he sleep,,carry him for another 15mins on ur shoulders and thn only make him sleep.
let him sleep elevated or in car seat.
6. often spitting is caused by overfeeding esp if the baby is on bottle,,so make sure to avoid that.
your baby at 3-4months should be drinking around :
5–6 feedings or on demand,,or 5–6 feedings of 4–7 ounces each,,iron fortified formula or bm with bottle...
try to offer breakup of these feeds further in smaller feeds if it helps you to get rid of the spitting up.

8.eat healthy diet if you are breastfeeding,,take diet that does not contain too much of hot nature foods or cold nature foods,,balance it well,if you are taking garlic for bm boosts,,make sure to eat good ghee and dairy with it !
please talk to your dr,in case of below :
1. it worsens and spitting is more often,,babies are fussy in burping.
2.ne temperature after burping or during burping,,
3.smelly bowels and yellowish urine.
4.baby feels fatigue after formula feeds and happens to sleep after burping.
5.if your baby requires burping for more than 15mins,he needs an attention from your dr.
6.ne colour other than white curd like,is sign of infection,,rush your babies to dr.
7.if your baby is sucking bottle for comfort and vomits during or after main feedings,,prob you want to minimize his feeding amt,,he is getting overfed.
8. decrease in weekly wght requires your dr attention,,
signs like wants to drink more but cant drink,,wheezzing,,cough, gaggin,screaming during feeds is all signs of gastric reflux,,so take care about it,,rush him to dr. further vomiting with shrill crying is GERD.
9.reduce your dairy servings, wheat servings or protein servings and check if thats causing reflux,,sometimes reducing these helps babies to get away with gas problems and refluxes...
10.some babies were better once they were on soy formulas,,due to less lactose tolerance level,,which is the last one to check on ! so ask your dr if your babies are severely spitting up or vomitng curd like content,,and if soy formula might help ! but thats the last thing !
try giving your baby mylicon as directed on the pack, if you are in US and see if it helps prevent spitting.
before feeds, give your baby enough tummy time and feed them 5min prior to thr feeding time,,will help them drink formula without sucking air or spitting might reduce.

Firstly thank you again for the detailed information. yes now it makes sense between colic/vomitting/reflux.
My twin1 used to be colic first 3 months. But now he is fine.
The medicine that dr. prescribed for the reflux was for 7-10 days.
I had a few more questions:-
1. AFter how many hours do you repeat the dosage of any medicine.
2. As mentioned on the formula packaging, you are not supposed to keep the prepared milk for more than half an hour.
In that case can I atleast keep the powder in the bottle (dry bottle) and then add warm water when required. The reason I ask this question is because when I visit the doctors it becomes difficult to prepare feeds for both of them.
3. At what age can they be taken out.
My twin2 does not sleep at night, have started his night massage and bath from yesterday as per your suggestion posted in your sleep section. Lets see how it goes.

if your dr has given your medicine to prevent reflux,AVOID giving mylicon or gripe water,,,unless otherwise your dr says its okay to give.
one medicine is enough to cure this prob ! ask your dr when you can expect the recovery with this medcine,,ne medicine that is for gastric reflux should not be continued for longer duration,,ask your dr about number of days or weeks you can expect recovery with this medicne...

Colic is persistent, unexplained crying in a healthy baby between 2 weeks to 3months of week,,sometimes it continues upto 5-6months..
your baby is colic if he cries for more than 30mins hours,nonstop,,,often in these episodes,,babies bellies look a bit swollen,abit hard and makes rumbling sound..the crying is shrill cry in pain mostly because baby experiences abdominal pain caused by spasm,obstruction and for this,, the cause of this condition quite unknown,,i didnt mention this earlier, because your baby is not crying or his cries last for long hours like 1hours -3hours !crying is not at all involved in your case !
the causes and symptoms for a colic baby are immaturity of digestive tracts,hunger or overfeedings, food intolerance,feeling sep anxty at higher levels leading to emotional stress,,formula or bm offered more than warm temperatures and overstimulation or overexposure to too much noise,light or excited atmosphere around the house..
often in these babiees clench thr fist,keep thr toes curled tightly and tensed,,you try to release thr fist or toes or legs,,they will keep it stiff..and these babies often like sleeping in D shape like legs bed towards chin or body,,
medications are unlikely to cure colic,,and this is just a phase that stays upto 3months or is often called infant colic or 3monthcolic...
massgng babies back with gentle strokes, holding baby in sitting position also helps to prevent colic..
sometimes mothers diet having dairy,beans, hard grains, caffine, citrus,nits, brocolls and potato,,gives colic...
In india, i will not be of much help to you regarding the bottles,,but in US we get age wise nipple..
your bottle nippes irrespective of place its sold,,is leaking because it needs to be changed,,whenever the bottle nipples leaks,is bit softer thn before,,and becomes discoloured,,its needs to be changed !
try looking for newborn or 0-3months nipples if at all you can find that in ne good babystore.
nipple hole size can be checked by filling a bottle with cold formula,,turn it upside down and count the number of drops released when shaken or squeezed...a nipple hole is of right size when it releases 1 drop of formula every second or at every tick of clock !
about mylicon, its main ingrediant is simethicone..see if you can find that in your store as over the counter infant drops,,or ask your dr to give you drops that has this ingrediant in it.its given to colic as well gasy babies and to babies who were vomitn because of gas being the reason...
also, alternatively, bonnison gripe water has also helped babies to reduce thr gastric reflux and gas caused by swallowing of air and stomach discomfort..please note that gripe water will stop discomforts related to gas,colic or stomach upset/indigestion and not constipation !
you yourself can eat things like ajwain, dill seeds, check my post in bowel and bladder board to panna dated 6th nov,,it has some tips to prevent gas..
your confusion is quite understood,,because mostly for all these probs,the symptoms are more and less same and so are the treatments to some extent,,,actually they all have diff root causes and to catch right one becomes difficult unless all the symptoms are shared..your babies are not crying for hours together so its unlikely that they are having colic..but yes they might be having spitting or now like you said vomiting/reflux,,which requires your dr attention.
it will take time for your baby to recover out of it,,keep him upright as much as you can, esp during and after feeds...

Thank you so much for the detailed mail, I really appreciate the time you have spared for this.
My elder twin threw up thrice in a day with higher thrust (vomiting). after speaking to the doctor put him on medicince. The voimiting reduced to an extent but continued. Now he spits after every feeds, even after burping.
Will try feeding only in upright position, and keeping him upright position after feeds for 15th mts. Will see how it works.
On the bottle front I do feel the hole on the nipples is a little big, cos it keeps leaking from their mouth during feeds. But these are readymade nipples and already have the hole in them. Do you get nipples as per their age?
What is the Indian version of Mylicon, cos I stay in Mumbai.
Can you tell me the symptoms of colic. I am a little confused with colic, gas, vomitting/spitting.
Will keep you posted with their progress.
