Dear all,
I am very thankful to you from your advices but My 7months old baby ‘s condition is still not improving.I tell you his whole history.
1.He was born in 36th week (He was due on 15th Nov but born on 14th Oct by an emergency c0-sec die to rupture of water bag & he weighed 2.6 Kgs at birth).He started gaining good weight as he was on Exclusive Breastfeed till 2.5 months but then we introduced Lactogen-1 with bottle because I was getting many calls from my office to rejoin asap.Till 3 month he was on BF & Formula but later on he started liking only Formula.Since birh,he used to spit milk like anything sumtimes even from his nose & Its not milk but it was like clotted curd which used to smell a lot. We went to his peadia & he said it is GER but nothing to worry as his weight gain was good.He strated suffering from diarrhea in the starting of 5th Month.Dr prescribed Bacigyl for 5 days & he was well.After 15 days again he developed loose motions & Dr said it is Viral Diarrhoea and didn’t prescribe any any antibiotic.I had been giving him probiotics like Reflora-R & Derolac etc & Iintroduced him Cerelac with Apple puree and Daal & on that evening itself,he suffered from Large watery motions ,I changed the Dr and met a new paedia,He prescribed Bacigyl again & sum probiotics & went for a stool test which did not show any pus cells and reducing sugar .My son was well after 5 days Bacigyl course but on that evening itself he again suffered from vomiting & smelly loose motions.We went to his Dr,he have him Ondem ,his vomiting stopped but loose motions didn’t.By Sunday morning he was so much dehydrated that we did not any option other than getting him admitted and giving IV Fluid with antibiotics.again a stool test was performed which found 2-3 pus cells & Reducing sugar.Dr said that it is a case of Secondary Intolerance & lactose takes 72 hrs for complete elimination from the body & we kept on waiting like helpless & seeing our baby passing large amount of water.After 72 hours ,losse motions are not stopped then Dr went for a stool culture which shows Shigella spp & Fungus is grown.Dr said Shigellsa is a very dangerous species of gram negative bacteria.My son was Resistant to most of the antibiotics including Amikacin which were given since his admission in the hospital.He was mild sensitive to Taxim & dr started giving him Taxim & Zocon for his fungal infection..He was on Zerolac(Lactose free soya formula) & after 5 days course he was better & discharged on Gramoneg.Dr told me that soon I’ll have to start with high protein diet coz his intestine lining has been damaged due to chronic diarrhea and I gave him Khchri with curd in night & upma with apple puree for his breakfast.He again started having severe loose motions & yes,he was on Lactogen since twice his stool test was showing –ve reducing sugar.He again he had to be admitted due to dehydration & given Gentamycin for 7 days.His condition was improved in 48hrs & he was allright by 72 hrs .We were preparing for returning back to our home but his culture report again showed Fulgal spores & Shigella grown but his sensitivity test was much better now he was sensitive to most of the antibiotics.He was again discharged on Gentamycin injections for 5 more days(as we have given 2 days dose in hospital )and Zocon total for 5 days.He was passing 2 hard motions morning evening as long as Gentamycin was continued but nowadays he is passing total 6-7 motions,1 of them is pure watery.My mom is saying not to give him Khichdi,it mst be difficult for him to digest & from last 2 days he is passing 4 motions a day.I am very tensed as his culture report has come today which is showing Shigella.grown with 2-3 pus cells no RBCs.
Motions he is passing is semisolid sumtimes solid & 1 watery but they are brown/greenish,foamy ,a bit whitish I mean they are not his normal stool,he passes.
Now pls resolve few queries of mine-
1.In the meanwhile,I met a very Sr Dr of Medical College Indore,he said it is simply a viral diarrhea and No Shigella.He said that our previous dr is unnecessarily admitting children as he is a partner of that hospital.His point seems to be logical also becoz he saud.â€If it were Shigella,pus cells count would have been in hundreds.
2.I think,he was not suffering from GER,it was some infections since his birth which made him vomit milk like anything becoz when I introduced Zerolac to him,his reflux is almost solved.He may be a bit lactose intolerabt since birth becoz throuout the night I had to feed him but he keeps on spitting milk & again hes hungry that’s the reason I had to give him bottle feed also although my supply was good.
3.I met another Sr Dr in the same hospital ,she is saying that his intestine lining is damaged so he keeps on passing motions for around next 2 months becoz absorption is not proper in intestines which leads to Reflux,ie,feed & motion.
4.Today again ,I went to his Dr with his culture report showing Shigella.He asked us to get his culture from sumwhere else.He was agreed to my logic that How can Shigella persists in the body without developing pus cells.
He said,it is an ongoing infection,source of infection is still to be given him,may be thru bottle nipples(I have changed all his bottle after his diarrhoeal episode),toys,may be my nails anything.Since no organism survives in the stomach for such a long period & after so many antibiotics.
4. I met a new pathologist today & he said .â€You ppl are wasting money.Just go for routine stool test,if it is shows pus cells,go for cultureâ€
5.I have been giving him sum home remedies like Bel Fruit(It has antishigella properties)and Isabgol etc.
Now this is the whole story which have made us mad.We both are working but the whole month we have to spend in the hospital & still my son is not recovered.Please share your experiences & I need esp suggestions from any pediatrician.one thing more,he was 7.4 Kgs on discharge on 26th April & now weighs 8.2 Kgs & eating Rice ,Sabudana ka starch,lactose free soya milk & 2 bananas per day.He is a very active playing baby but yesterday & afternoon he suffered from fever.
