I went and visited the RE. He asked about my history and diagnosed me with PCOS. He also put me on birth control pills to induce my period. He says according to my previous tests, all my hormones are normal but the LH level in my body is higher than the FSH. (I don’t know what that means!) He will take a number of tests (HIV, STD, hormones etc.) He told about 15 test tubes of blood. He will also do my husband’s sperm analysis. He has also asked me to take the HCG test. He will put me on injectibles to induce ovulation. He wants to go in for IUI next cycle. I personally think it is too early. He says with successive tries, the chances of conceiving will reduce.
My question is should we go in for IUI the next cycle or wait for a couple of cycles only on injectibles. Its been 10 months since we are ttc. Is it too early for IUI and should we try on our own?
I am getting anxious and nervous and am waiting for your opinion on this.
Thanks in advance,
Lubs.

sorry i could not reply earlier ,
good to know that all the tests have been done ...if your LH is higher in the follicular phase, it is going to inhibit follicular growth and hewnce affect ovulation .....
i have been through IUI and i personally feel that it was not too bad and not too stressful ...
however IUI has a better success rate than IC when sperm count is low, when your mucus is hostile(they do a post coitla test to figure that out) or when u have multiple follicles ...
so the decsion of IUI or not is upto you .. but be sure to track your ovulation thorugh u/s even if u r trying naturally as that would be more accurate ...
good luck girl, try not to get too tensed abt it and hope this is ur month :) ...

Lubs replied. Thank you Madhu for your reply! My RE has asked me to start taking injectibles. I told him I was not comfortable wtih going for IUI so soon and wanted to try on my own with injectibles. I am scared and tensed. But thanks very very much for your help. Your reply has given me some relief.
Madhu replied. Hi Deepa,
First of all, do not be sad. There is always hope.
Your fsh is slightly high, also your lh is high. Your u/s scan too has showed poycystiv pattern, so yes u r PCOS.
U do not have to jump to ovarian drilling option. Find a good reproductive endocrinologist who will study your hormonal profile and bring your hormones in balance with the help of fertility drugs. Try the fertility drugs to help u ovulate. IF AFTER REPEATED ATTEMPTS OF ANOVULATORY CYCLES WITH FERTILITY DRUGS, U CAN GO IN FOR OVARIAN DRILLING. PLEASE DISCUSS WITH YR DOCTOR.
Laparoscopic ovarian drilling is a surgical treatment that can trigger ovulation in women with polycystic ovary syndrome (PCOS). Electrocautery or a laser is used to destroy parts of the ovaries.
This surgery is not commonly used. But it can be an option for women who are still not ovulating after losing weight and trying fertility medicines.
You are still young and not overweight , so there is lot of hope for u. So find a good RE , discuss with him. Do not jump to ovarian drilling option.
Please weigh the pros and cons before coming to a conclusion.
All the best.

Madhu replied. Hello Lubs,
PCOS means polycystic ovarian syndrome. In PCOS candidates, LH(Lutenising hormone) is greater than FSH ; if the the ratio of LH TO FSH is greater than 2:1 then u r treated as PCOS candidate. IN PCOS candidates, there are good eggs(follicles) but they do not mature enough so as to rupture (ovulate) on their own because of this imbalance in fsh and lh.
Also check with your RE whether your fasting serum insulin is on the higher side. If yes, then u might also need METFORMIN tablets alongwith ovulating induction drugs. METFORMIN alongwith ovulation induction drugs over a period of time is sureshot recipe for getting pregnant in pcos candidates.
Blood tests for HIV, HEPATITIS, VDRL , RUBELLA etc. and various other hormones like T3, T4, TSH, fasting serum insulin etc is one time and is good that it is being done so as to rule out any infection or imbalance in hormones before u start iui or get pregnant on your own. After seeing your blood reports he will be able to advise u the proper treatment protocol. So do not worry. Its normal.
Husband's sprem analysis is a must. VERY IMPORTANT THAT YR HUBBY DOES IT as it will give an idea about the sperm count, morphologhy of sperms, motility etc etc.
U HAVE MENTIONED ABOUT HCG TEST, I THINK WHAT U MEAN IS HSG TEST WHICH IS VERY IMPORTANT TO KNOW WHETHER UR FALLOPIAN TUBES ARE OPEN. PLEASe READ MY RESPONSE TO A QUERY OF VIDHU DATED 12/10/2006 ON THIS BOARD WHERE I HAVE EXPLAINED IN DETAIL ABOUT HSG TEST.
There is no harm in going for IUI
AS IT WILL BYPASS some of the factors of unexplained infertility if you have any.
But discuss with your RE whether U CAN DO IUI with ovulation induction tablets instead of injectibles to start with. Then later may be u can move onto injectibles with IUI.
PCOS patients take a little time to get pregnant . They have to be patient and sincerely follow the treatment protocol.
Hope this reply clarifies your doubts.

We have been married for the last 12 years. We are trying for a baby but unfortunately every time it's disappointing. I had 3 abortion, 2 Ectopic pregnancy, 1 IUI failure and 2 IVF failures. I had septum in the uterus and got removed. Since I had a history of ectopic pregnancies, both my fallopian tubes are tied. Due to the poor ovarian reserve ... - Malini [View Message]
