Hi Archie,
I am trying for a 2nd baby and i am 35 yrs old.We are trying since last 8-9 months but no luck and i got my priods irregular.I consult a Gy/Ob and she told me for a blood test and ultrasound.I went through these test and test came normal.My Pelvic Ultrasound and Transvaginal reports are lik these.
Uterus normal sized retroverted without fibroids.normal 7.7 mm endomterium.
Both ovaries are normal sized with numerous peripheral follicales in each ovary.
Comments:: There ar several less than one centimter sized peripheral follicals in both ovaries,which number mor than six.This is slightly more than usual.There is no free pelvic fluid.
Can you tell me more about my report.Is it looks normal.Is everything is ok??
In first pregnancy i didn't face any problem to concieve.Now she is suggsting me to take clomid 50mg to boost the chance.I just want to ask you that should I go for clomid???
And how is my report???
Pl.help me.Thnaks in advance.

Sorry about delay.
Looks like not a big concern on your report.
Lets first clarify one old tale that once you get pregnant you should not have problem getting pregnant on second time: this is untrue. Regardless of how many time one got pregnant and carried to full term the chances and fertility issue never changes rather with subsequent pregnancies if that is after a substantial gap and not following deliver of previous one can take sweet time than first pregnancy. Mainly because the age factor complicates matter. Besides when you don't try right after deliver (whenever you start menses) body senses to go back to normal and therefore it would take as long as a year or more than that to get pregnant for the second time.

What I would have loved in your report is analysis of
Prolactin,
Estrogen and of course
Progesterone if possible.

Since you are a mother of one I would want to know if your Prolactin levels have gone back to normal after you stopped nursing your first one. If the levels are elevated then you may just need to treat Prolactin levels and bringing them down will help you ovulate regularly.

Estrogen is produced when follicles starts maturing. Looks like the stage (the day you have had u/s) follicles are not matured. In order to ovulate your body must start producing one of the mature follicle which when starts producing estrogen boosts your basal body temperature and is a strong sign of ovulation.

following to this your Progesterone levels must match to Luteal phase regardless of pregnancy or menses are in store for you.

If these three players falls at their respective places then I would suggest you to go with Clomid otherwise I don't see a strong reason.

True that if your ultrasound was done near to your expected ovulation (which I think your Dr. may have taken in to account while choosing the day) then your follicle size indicate failure of ovulation. This is why instead investigation she may have thought of putting you on Clomid. However, I know clomid can not make miracle for everyone, one would have to go on increasing dose of the drug for as long as six cycles and failure will lead to extensive elaborative methods which might not be needed if your basic reproductive endocrinology is made clear.

I don't have anything against clomid use in your case but I would have loved to have hormones checked before treatment.
