For a young toddler:
Morning (on waking): Milk (5-8 oz)
Breakfast: Sooji/Nachni/Oats/Idli/Cereal. Let her have whatever she likes. If she doesn' t like oats, just omit it from her diet. Add a little ghee, 3-4oz milk and a bit of sugar to her breakfast.
Mid morning: Milk + fruits/biscuits/eggs/french fries/carrot sticks/etc.
Lunch:Rice/Roti + Veg/Chicken/Fish/Eggs or instead you could just give her pulao (with 4-5 veggies chopped fine and cooked with the rice + some chicken stock)
Evening: Milk + fruits/biscuits/custard
Dinner: Same as lunch
Before going to bed: Milk (5 - 8 oz).

She is too old to be having mashed food. Try giving her partly mashed food (lumpy and soft), and after a month or two, just offer her small bite-size chunks of well-cooked veggies and fruits, bits of roti, well-ccoked rice, etc.

About quantities of food - she should be having approximately 14 - 16 oz of milk daily, other food varies from toddler to toddler, but offer her as much as she wants. It could be 1 full aloo paratha for a meal to 2-3 chapatis with some cooked veggies and chicken. Let her appetite guide you.

Check with your doctor if you can introduce appetite stimulating syrups (like Polybion-L or Mimorange-L). This can be given twice a day for a month, and it helps increase the appetite. But, please check with your doc.

If she is shy, you have to increase her exposure to other people (both kids as well as adults). What you can do is take her to the park once a day without fail. Let her meet and play with other toddlers. Initially, she may be reluctant, but eventually, she will want to play with them. Take her over to relatives or friends houses and let her interact with adults and kids. But, if she is shy and clings on to you, don' t chide her. Just let her be. Gradually, she will learn to socialize. Eventually, you can enrol her in playschool where she will interact with kids on a regular basis.
