Dear All,
I have been silent reader of this board and have been consoling myself whenever feeling low by reading beautiful heart touching messages of Sunshine and all. The way Archie finds time and answers all queries is remarkable...Sincere gratitude for that...
Please read through my long story...
I am 29 yrs old, my husband 31 yrs married for last 3-1/2 years. In Sept 04 I was detected an Ovarian cyst and was getting treatment for that, also started for conception as doctors suggested that as cure. So we have been TTC for last 1 year. We did USG monitoring for two cycles and later doctor suggested HSG. In Aug, my HSG report wasnt good and showed blockage in my right fallopian tube and I underwent Laproscopy in Sep 2005. Laproscopy confirmed my tubes are patented however Moderate to severe endometriosis was diagnosed after biopsy. Doctor had punctured the cyst, but after that I wasnt put on any medication like Lupron etc... I was on Clomid for two cycles with USg monitoring, however no success. Recently I visited two top doctors in Mumbai for their opinion, and they are quite different. One of them has suggested to go for IVF straight away before endometriosis recurs ( as per him IUI is of no use with this kind of endometriosis), while the other suggested for 2 cycles of IUI before IVF on heavy dosages of ovlution induction drugs. My periods have always been regular and painful (now I know the reason of pain :-) and in all the monitoring I have ovluted either with or without HCG injections.
My FSH/LH are quite normal however Prolactin level is 26.5 (as per the lab report normal should be less than 25). I have had very darkish brown discharge in last cycle two days before period, not sure if that holds any significance. My Husband's Semen analysis reports are normal.
Wanted to know if anyone has any similar experience, and suggestions whether we should go for IUI or IVF ?

Nothing else in Life ...Just Hope :-(

Dear Anu,
Endometriosis is one of the cause of infertility.

painful period is not always indicationof Endometriosis, although it fits in your case.

Anu, what would be nice to know is how much of heavy endometrium did ultrasound estimated? Do you get in between period bleeding that puts you in trouble as to you have menses or not?
Do you have any abnormal feelings in routine life, fatigue, sweating, heat feelingd etc!

How do you know about ovulation otherwise?

Did you ever noticed Cervical Mucus while around ovulation?

These answers would help understand little more about your situation.

The treatment would vary from case to case. Of course for a mild case superovulation drugs would help, in some cases IUI might be needed.

However, other than endometriosis I have noticed more analysis is done and tubes were patented. Have you any idea if they are in OK after hysteroscopy? You may want to talk to the Dr. who performed hysteroscopy.

I understand second opinion always matters.

In your case, indeed both experts advise same thing, not radically different. It is just that one has some confidence that your case isn't that bad to go directly for IVF but other wants to be on safe side. To choose between IUI over IVF would your your economic decision.

I want you to know that both processes have chances to fail. Although success is more likely with IVF than IUI. Given your spouse fall in normal range, IUI can do the same job.

I'm sorry to hear about your pain... I can only understand the mental torture that u are undergoing. However, since u have been to top two fertility specialists in Mumbai, have you seen Dr. Jatin Shah? I suggest u see him if you haven't.

Archie would be a better option to address your query on whether u should go for an IUI/ IVF.

Thanks dear for your wishes...You have been an epitome of positivity and I would try to emulate that :-), my sincere wishes are with you. I havent met Dr. Jatin shah, but doctors in Lilavati. I will try and meet him as you suggested.
