Breastfeeding:urgent - help needed

my baby is 4.5 mts old and im breast feeding exclusively. Yday all of a sudden i had a plugged duct and my breast became so hard.. [i didnt miss any feed.. my usual feed got pushed to 2.5 hrs as against 2 hrs as my baby was sleeping] and i saw a small white head it one of the ducts.. i tried hot water massage etc and even pumping nothing worked. my baby refused to nurse. Then i went to the doc and she tried with cold compress and released all the ducts except 1. then she had to pierce it with a sterile needle and open that duct. After which i expressed all the clogged milk out. but from last night and this morning i feel that the duct has closed again. as when i express milk sprays from all the other ducts except this one. im really worried.. anyone with similar experiences? any after care treatment? i was all set to start solids next week and now im scared.. pls help

keep ur breasts clean.
message them with warm water to realease milk ducts.
feed ur baby frequently.wipe everytime breasts b4 feeding.but first go and c a dr so that inf wont get worse.
