Infertility Solutions:Need urgent advise before 2nd ICSI attempt
2008-07-28
Name: Pleasehelp

I wld like an answer to my query.
Recently i had my first IVF-ICSI attempt which failed .(NO IUI before as in my case it is the problem of low sperm count so Dr. said that IUI useless only IVF).
Since i did not have any problem at all my Dr. was very confident that it will be sucessfull in first attempt .When it failed then the Dr. told me that the possible reason was since i did not have any fertility treatment -like IUI before my body was new to this & since embryo is like a foreign element in the body the body cld not accept it .But said that if i go for a second attempt with in 3 months then chances are bright as the system will accept the embryo as the body is already use to this process.
But on the other hand my friend who has had IVF said that after ET(which is done on full bladder)i must have passed the urine forcefully this lead to the embroys coming out of the body .
Please guide me as to who is correct the Dr. or my friend .And shld i go quickly for the second attempt .
I need sincere advise & hope anyone who had a similar experince will guide me as i do not want to take any haphazard decision as each attempt cost a lot & we do not have that much money that we can spend one after the other .
thanks in advance

Hello! I underwent 2 IVF-ICSI failed cycles at Dr Kuldeep Jain Delhi. In my first cycle ET created lot of problems as dr says my uterus was antiverted and he was not prepared with the instrument for doing this, so he did the ultrasound guided ET by filling my stomach with glucose and after that made me urinated there. So i don' t think urination can be a problem.

Dear Pleasereply
It has been some time since you posted, so perhaps you are no longer looking here. I have a lot of exerience in these matters. It is definitely not possible to pass the embryos out from your bladder. So unfortunately your well meaning friend is wrong.
However, the prevailing wisdom on ivf is that each cycle is unique. Therefore, if you got fertilised eggs from your icsi, it should be possible to re-create this. How quickly you do or don' t do this is not important. You must feel positive and healthy. There are lots of other factors that you should also consider each of which will determine your success rate next time.
1 the thickness of your lining on the day of ET (should be min 8mm, ideally 10mm.
2 the quality and number of embryos: at least 8 cells and symetrical.
3 the level of essential hormones oestrogen and progesterone.ecah of which can be given orally, nasally, patch and via injection depending upon the need of the patient.
4 the number of eggs you produced, the number that were fertilised etc.
If you are trying again, perhaps you could check the above.
Obviously the other main reason for not conceving is whether the embryos are genetically perfect. Any imperfections will typically result in a negative test or a mis-carriage.

I hope this helps. Wishing you all the best.

Dear parveen,
After a long time checked this site today and found ur answer to my query .
thanks a lot .
I went through all the points written by you .
My each and everything is absolutely ok the only problem that happened was i did not get enough eggs(only 2 .both embroys formed one good quality one poor quality both were transferred ).I get very good size and quality eggs in my natural cycle so my Dr. says whenever i go for the next attemp she will not give me progynova and lupride injection .Will put hormone injection in natural cycle only .Let us see if it works.
thanks
Please help .

As KM suggested, I would urge you to go with the doctor on this one. The blastocyst is usually placed in the uterine cavity with a properly prepared endometrium. Many centers use an ultrasound to ensure the correct placement of the blastocyst. This being the case, the embryo cannot be urinated out of the system.

You need to keep positive and believe in the process. Be positive.

PS: Where are you getting your treatment?

Hi pleasehelp

go with your doctor' s suggestion. embroys doesn' t expel from the body by passing urine forcefully. don' t worry about failed ICSI. fertility is so complicated issue and nobody knows why it is unsuccessful. you may go for several fertility treatments and get negative results but you may suddenly conceive naturally. that is what happened to me. but we can' t keep our fingers crossed and wait so we have to take technical help. my advice is go with your doctor. even if you fail a couple of times don' t get disappointed and feel guilty that you have done something. first try to understand the complexity of fertility issues and be positive. believe me being positive and happy helps a lot. by the way where did you get your treatment?
I hope this advice will help you.

Thanks a lot for the answer .It gives me a lot of strength & positivity .Anyways I am keeping my fingers crossed & hoping for the best .
I have faith in my Dr. she is very detailed oriented & compassionate also .She also told me when i met her last time to be relaxed for my next cycle .I am a very positive & strong person by nature but somehow taking treatments for 9 years took a toal on me & i think i got tensed the last time .But will be careful this time .Plus since i know the process fully i will not be nervous this time .
Presently we are based in Coimbatore so taking treatment here .
Thanks a lot once again for the support .
