Routine Tests:archie cervical swab results

hi archie
i had told you that i was suffering from funagl infection from 3rd month of pregnancy. in april i went in for a cervical swab test, whereby it was detected that there was presence of enterococcus species in the vagina. i was thereafter treated with gention violet vaginal paint for 8 days along with insertion of candid V6 tablets at night. however, since the infection was still there after 8 days (though it had reduced around 90% acc to dr), i was adviced to continue with Candid V6 tablets till 36 weeks got over.
last week i completed my 36 weeks and the dr did another cervical swab test. the reports have come in and they say that no fungal elements have been seen. but it says that on Gram Stain- plenty of pus cells, few epithelial cells, gram positive bacilli, and gram negative bacilli. the report also says - no aerobic growth after 48 hours incubation.
what does it mean? will i be able to deliver normally, or do i have to go for caesarian section? i am worried as i do not want this child desperately esp since i lost my last one.
does my high sugar level of 157 PP have anything to do with it.
i had also gone for a Urine Culture 15 days back which came clear.
please help and throw some light......

dear Ash,
I remember you with history and whole of this term.
I know you want this baby desperately. I also know this baby is a new hope for you to lead path in future.

Having said above, I shall get on business.
the description of Gram Stain indicates presence of bacteria in vaginal path and are of mixed type, both Gram positive and Gram negative. Although your streptococcal infection is cleared after gention violet treatment and that candid took care of fungal infection your vaginal path is not really safe for baby to take birth.

Remember your last loss was dreadful and it was due to septecemia. I remember explaning the demise of your last baby in medical sense. In order to prevent septecemia in this child I would have to let you elect a C-section over Vaginal delivery. I am always for for Vaginal birth but in your case knowing whole pregnancy and last loss and most importantly reason behind loss I would not encourage you to try for vaginal delivery with mixed vaginal infection. Although this infection is not bothering you, it will infect your baby during birth through vagina. Here in USA there is no antibiotic treatments for nacent born and you will be in trouble once again.

In order to bring this baby safe and sound with good health in this world you will have to prepare your mind on C-section and based on present weight of the baby you could choose to deliver it either in 38th or 39th week. Do not wait for natural labor or even if you are in labor let the birth happen by C-section.

I know you will have more to bear for 3-4 weeks after surgery but once your baby starts smiling and developing your all stitches will sooth and you will feel proud of your C-section choice.

Good luck
Archie
