Dear Archi

I recently came down to my mothers place for the delivery due in october. Here the doctor asked me to get some tests done. The following are the results of the same:-
1) TORCH IgM profile :- in this the antibodies to cytomegalo virus is coming out to be equivocal(needs retesting)toxoplasma, rubella, HSV all are negative
2) TORCH IgG Profile :- In this antibodies to toxoplasma is negative, antibodies to rubella is positive, to cytomegalo virus is positive and to HSV its positive.
3) In Thyroid function the thyroxine(FT4) level is a litte low 0.63 as against the normal 0.71-1.85 reference range. In thyroid profile done in May the T3 level was a bit high 2.63 nmol/l as against the reference range of 0.92-2.33

My appointment with my doc is two days fm now. What i wanted to know is how would u interpret the result of the TORCH test and should the slight fluctuation in the thyroid levels be of concern? After my first thyroid function test in MAY i consulted the specialist and he said its normal to have a slightly increased thryoid level in pregnancy. (I donot have thyroid problem in my family)
3) My doctor has asked me to prepare a chart for the baby movements. What i wanted to know is how is one movement counted? baby moves once..is it counted as one movement...or a series of movements say in 5 minutes wuld be counted as one movement.(i know i sound confusing).

My Amniotic fluid level at 30 weeks was 17.9. The doctor said its high. Here the other doctor says its normal. What shuld i make of it?
Rest all the tests are normal. The blood culture, urine, GD everything is normal

I wuld appreciate if you could throw some light on the queries.
thanks and waiting for ur reply
love

Hi chetna,

I would suggest you to get the test repeat once again from Dr Lal path lab center near to ur place.I also had these torch IGM's equivocal or slightly posotive and even thyroid was also posotive (borderline posotive). First time i got these test done from capital , i guess. then my doc asked me to repeat those from Dr Lal Path labs only and when i got it one again all my reports were normal. She told that earlier also she has noticed some errors in the reports for other clinics.

so have ur tests done again.
Also I was going thru archie's comments..actually my doc said that these tests are done twice once in first trimester and secondly in third trimester. so chk with the doc for that..

good luck!!

Chetana,
your new Dr. seems thorough but what I want you to know is rate of C-section done in his/her clinic.

I don't see many tests as of concern because they are most required in first trimester and now you must be somewhere close to term and studying them now won't change prenatal care. If they were not undertaken in first trimester and you faced no immunological or viral complications youa re good to go for delivery.

AFI is absolutely normal and I must say you have nothing to worry on that. I don't see it as a high value at all. If it was above 20cms I would have suggested repeat of sonogram and if it was above 25 then suggested some cautions or questioned on your lifestyle and diet. If it was above 30 then may have suggested immediate delivery. Becuase it's 17.5cms you should be doing just good. Besides in third trimester AFI value are known to fluctuate.

about movements: you can count them when baby is moving. If baby starts moving and moves at least 10 times in 2 hours from the time it started to move then all is well. for some 10 movments in 2 hours at any time of day are considered to be normal. If there is some concern on this part then you may be offered NST to note baby is doing good.

What's exact TSH value at this time of pregnancy? Slight elevated or reduced values are not necessarily be alarming signs. Besides you have carried this pregnancy without thyroid concern you should have a word on this with your previous OB, who might answer your query to your satisfaction, to start or not treatment and at what number is individual choice some Drs. make.

As I said earlier this minor fluctuation on thyroxin is not going to alter prenatal care to great extent you should not be worried.

you can get tests repeat either at same lab or somewhere else as a second confirmation.

Dear Archie

firstly thanks a lot for replying back.

The doctor i am going has got a lower rate of c-section. My both sisters had their deliveries at her hospital.

I had got all the tests done in first trimester except for TORCH which i got done a week back. Thyroid profile i have got it done twice, once in second trimester and the second time a week back. In the last Thyroid function(done a week back) the thyroxine(FT4) level is a litte low 0.63 as against the normal 0.71-1.85 reference range. In thyroid profile done in May(second trimester) the T3 level was a bit high 2.63 nmol/l as against the reference range of 0.92-2.33

Lets see if the doctor wants to repeat the TORCH profile again.

thanx again for being so helpful.

take care
love & luck always
