Intra-Cervical Insemination - ICI

Intra-cervical insemination or ICI is the procedure by which a woman is able to conceive. This procedure is akin to artificial insemination. Insemination is a tedious process. It can happen in different ways. Read on to know more about the procedure.

ICI or intra-cervical insemination is similar in concept to the IUI or intra uterine insemination but different in procedure. This is an artificial insemination process that can help the woman conceive a child when the couple has certain medical conditions that bar her from experiencing the joy of motherhood. In this process the sperm is artificially placed inside the vagina of the woman. Insemination happens in four ways:

  1. Vaginal insemination
  1. Cervical insemination or ICI
  1. Insemination inside the cavity of the womb that is the IUI or intrauterine insemination.
  1. Insemination inside the pelvis that is at an area adjacent to the tubes and ovaries. The medical term for this procedure is intraperitoneal insemination.

Barring the first one, the other three are artificial procedures and nowadays in majority of the clinics the husband or the male partner is asked to be present in the room where the procedure takes place. This way he can feel connected with the woman during the insemination process.

How is Intra-Cervical Insemination (ICI) carried out?

Production of Follicle -

In ICI, the insemination is carried out during the natural or the stimulated cycle. If it is through a stimulated cycle, it will require the administration of fertility drugs in order to trigger the stimulation of the ovaries of the woman. The objective is to cause the production of two or three follicles instead of the usual one follicle.

Educating Patient -

Physicians always make it a point to make the patient completely aware of the risks involved in this hyper-stimulation process. This means that there are possibilities of the woman experiencing certain discomfiture and side effects that are associated with the process itself.

Preventing Risks -

As a usual practice it is found that woman who experience normal ovulation cycles, require an offering of four to six donor insemination cycles. In such cases the entire process of medically stimulating the ovaries can be done away with. This in fact is considered perfect as there is a natural reduction in the risk of the formation of multiple pregnancies. Thereby the other associated risks of such a situation are also safely avoided.

Examining the Cervical Area -

The Intra-cervical Insemination as a procedure is quite painless and is a simple one. The patient or the woman is asked to lie on her back on the examination table at the clinical laboratory. The procedure is carried out by the physician or the nurse and as said earlier the male partner is often allowed to be present during the process. At first the nurse inserts a speculum inside the vagina in order to visualise the cervix and the cervical area.

Insemination of Sperm -

Then the sperm that is a frozen state is thawed. It is placed under sterile conditions inside the cervix or the neck of the womb of the woman. This entire action is carried out with the help of a fine plastic catheter that has a syringe attached to it as well. At times the physician might prefer to use a sponge or a cap inside the vaginal wall prior to the removal of the speculum.

This procedure is done to ensure the positioning of the sperm near the cervix. The patient is advised a short rest before leaving the clinic and the couple can even carry out intercourse soon after the procedure. After a gap of six to eight hours the woman is instructed to remove the cap at the home.

ICI is carried out at the ovulation time. The timing is determined through blood and/or urine and hormone tests to monitor the LH surge. Ultrasound scans also help. The insemination can be carried out once or twice every month. If pregnancy does not result after 6 cycles, other treatment options might be considered. Pregnancy test is performed a couple of weeks following insemination.

ICI gives hopes to many childless couples. I never heard about this procedure. I thought IUI is the most common procedure.
