In Vitro Fertilisation

While modern technology is very good at making embryos in the laboratory, we still cannot control the implantation process. We do not know which embryo will become a baby - and this can be very frustrating, for both patients and doctors.However, you need to remember that embryo implantation consists of a series of phases in which the embryo has to appose and attach itself to the maternal endometrium and invade into it.

The enigma of embryo implantation - why doesn't every embryo become a baby?

While modern technology is very good at making embryos in the laboratory, we still cannot control the implantation process. We do not know which embryo will become a baby - and this can be very frustrating, for both patients and doctors! 

Many patients who do not get pregnant after an embryo transfer start believing that their bodies are defective, and that they have "rejected" the embryo. They feel that if they failed to become pregnant even after the doctor transferred 3-4 good quality embryos, that they are flawed. However, you need to remember that embryo implantation is a very complex process, which consists of a series of phases in which the embryo has to appose and attach itself to the maternal endometrium and invade into it. 

First, the embryo has to undergo further development, till it reaches the blastocyst stage, when it hatches from its shell, known as the zona

The hatched blastocyst then needs to implant in the endometrium. This occurs during the period of time known as the implantation window. 

Apposition, or orientation of the embryo (which is at the blastocyst stage at this time) within the cavity of the uterus, starts when the cavity has become minimal due to the suction of endometrial fluid by pynopods (small protrusions found on the surface membrane of the cells lining the uterus). 

Adhesion of the blastocyst is a progressive phenomenon that ties the embryo to the endometrium and is the primary event initiating invasion. Many molecules, such as cytokines, growth factors and cell adhesion proteins called integrins play an important role in this complex process during which the blastocyst and maternal endometrium must undergo an exquisite dialogue. 

Invasion is a self-controlled proteolytic process that allows the embryonic trophoblast to penetrate deep into the maternal decidua and to invade the endometrial spiral arteries by producing chemicals called proteinases. How implantation is regulated and brought about remains an enigma, but we need to remember that the implantation process is surprisingly inefficient in humans - Nature is not always very competent! After IVF, it's only about 10%, which means that only 10% of embryos implant successfully to become a baby. The responsibility for this low efficiency has to be shared between the embryo the endometrium, as well as a defective embryo-endometrium dialogue. We still cannot successfully predict which patient will get pregnant after embryo transfer. We now know that one of the major reasons for failure of the embryo to implant is a genetically abnormal embryo. Basic research on implantation is of great interest today, because embryonic implantation is the major limiting factor in allowing pregnancy after ART, but we still need to learn a lot about this "black hole" in our knowledge, before we can learn to control it ! Some clinics attribute failure of the embryos to implant to immunological problems - and even offer "treatment" for this, but there is no proof to support this theory. 

Remember, embryo implantation is a complex process, which you cannot influence by your diet or physical activity, so there is no need for you to blame yourself if the embryos do not implant.

It had been 14 years of marriage. We tried numerous centres & doctors across the world but none was able to make us happy. We met her and the doctor was unbelievingly giving more satisfying answers and was having huge amount of knowledge. We have a very good experience with Craft Hospital, Kodungallur & very good opinion about Dr.Mohd. Ashraf, who has given our life back. We are married 10 years ago & we realized the normal pregnancy was a nightmare for us. We consulted many doctors & had undergone 3 IVF’s ( 2 in India & 1 in Saudi Arabia) but no result. In the year 2009 one of our friend recommended for Craft Hospital & we have gone through their web site & decided to meet Dr.Ashraf with our old reports. At the time of our first consultation Dr.Ashraf had explained about HLT (Husband Leucocytes Transfer), its success rate, time duration, etc.. & we have decided to go for that. We took 7 injection totally ( 5 before pregnancy –bi weekly & 2 after pregnancy). Out of the last 2 injection, one was taken at the time of first day when we got the positive result & second injection was taken at the time of first scanning. By the god’s grace & as a result of our tearful prayers my wife is 5.5 months pregnant now & thanks to Dr.Mohd. Ashraf & his team. We are strongly recommending to Craft hospital for those who have not blessed so far.

'Craft'Hospital, Kodungallur, Kerala, India, has accomplished its name internationally, leaving the question to many 'how'? I did not understand how this disorderly management gained this accreditation and acceptance. I am least convinced about the efficiency of management, the herbal medications, the cleanliness and all the more 'the unskilled and easy going employees' there. It is your decision time: 1.This hospital asks you to wear a 'lungi (used by many)' before you go for a scan! And pathetically the scan table has yet another USED sheet! Has this internationally renowned hospital not aware of hygeine? and the use of tissue roles? 2. Your are not supposed to use shoes inside the hospital and therefore kept pair(s) of sandals infront of the toilet and most of the time those sandals are missing and you end up using the toilet bare-foot! Note that you should carry liquid soap/soap tissue as I could not find any! If you ask for soap the workers would come up with the hidden liquid soap! 3. Most of the time doctors will not come on time. I am aware that they are involved in various responsibilities and do not want to disrupt them. However their duty and responsibility includes making an alternate arrangement or to have a time line for consultation. The management should realise that time has equal value for everyone and unnecessary waiting COULD be avoided. 4. None of the doctors discussed about the course of treatment or the cause and effect of herbal medications! Could not get any clue about most of the medications, apart from they are very expensive; 999/- per bottle. It is a patients right to know about authenticity of the medicines they consume. Altogether I am confused and not convinced about the hospital! ------- Messages from other members about Craft Hospita: Name: Anny Subject: never goto Craft ivf Date: 2010-10-24 i have very bad experience in craft. they don't have proper hygiene. The nurses don't use any gloves when handling the blood and they don't mark the name of the patients properly on the sample. The main dr is available only 15 days in a month. They are very very money oriented. They will give you unwanted aurvedic medicine for all of them and they makes these medicines themselves. I had done all my investigation procedures at other places and still they wanted to do it again just for the money. They even don't hold records of the procedure properly. It seems all the dr don't know what the others have done. They don't keep print out reports. If you see the receipts they give it looks like the one in grocery shop. It is a third class facility. after ivf procedure they ask people to share beds with others. The accomodation does not have even hot water facility. Also after each surgical process you have to buy them the injections and drip they used during the procedure, normally we think when we pay 15,000 that includes all. Go at your own risk..

I am currently undergoing IVF with Dr Archana Dhawan Bajaj. She is the best doctor I have met (and I have met a lot of doctors) She is very good natured and a good person and a brilliant doctor. She is in Adiva Hospital in Uday Park in South Delhi. I recommend her to everyone who is suffering from infertility. Please do not ever go to Phoenix Hospital in GK2 for infertility treatment. That place is very commercial and very poor in diagnosis.

They did not have proper investigation techniques, and during ultrasound scan each person has to reuse the towel which had blood stains as the scan was done on 2nd day of the period. Hi Blue Dr. Anoop Gupta is definitely not a place to go.He is just making bloody money there.My friend suugested Moolchand Hospital in Lajpat Nagar to me.Very good doctors,cooperative staff and my treatment has already started for an IVF cycle due in October-November.I say,its a must visit centre for your treatment with them.

1)blue i have been on web for the last whole week and found out dr anoop gupta (bangali market) is a money making man. comments said that you are treated very bad by the staff. somebody had a good experience from dr archana dhawan (naraina vihar). i am planning to go at 17a lajpat nagar-4 its so hard to decide for people like us travelling from 15000kms with hope. 2) cindy where did you do your ivf and how much did it cost you? wishing you all the sucsess and good luck to you

i havedone ivf once. 8eggs had foundbut only 2 eggs were fertilised but grade2. it could not be successful. this cycle i m going to do another ivf. there r 11 eggs.5 are 15 in size,2 are13 in size,4are 12 in size.et is 9.e2is 1285. is my report is ok.

just finished first round of ivf with icsi-sp. now just waiting the 12 days to take preg. test.two embryos were suitable and transferred after starting with 5 eggs.(kind of low response to stimulation). i also have a uterine septum. both embryos were placed on my right side as the left was difficult to reach.anyone else out there with a septum going thru ivf?..(:
