Name: sadique My wife is on her 30th week, few days ago she started frequent pain on pelvic area. Doctor confirmed urinary tract infection. Afte culture she prescribed for Cefloxin 500 and Urispas tablets. We are highly worried about antibiotics during pregnancy. But doctor says there is no option other than antibiotic.
Can any one help us.

Dear Archie,
Hope you are doing well. After my last message posted here, she informed me that pain had gone after 5 days course of Sporidex. Eventhough she didn't ignored it as she used to drink 3 litres water daily and followed the daily routine as per the advice of her doctor and your's through this forum.
But yesterday morning she started severe abdominal pain followed by back pain. Afternoon, afternoon she felt a little relief so that she didn't go to doctor. But same happened today so that she went her Gynec. After urine test doctor detected Severe urinary infection. Doctor was also upset due to this prolonged infection. She prescribed Urspas, Ceftum250 & Oricitral Candid. Also told her to stop all other tablets she currently taking(Actiferrine&Calsus etc). She called me with desperation eventhough she has taken care of everything as much as she could.
Her EDD is end of this month(30/05/2004).
Dear Madam, I hope everything will be alright? isn't it.

Subject:  Tips to avoid future troubles.
Dear Sadique,

I did what I could, nothing great. Anyhow you are more than welcome I just want her to go through normal pregnancy phase and deliver healthy baby.

Hope she is feeling better and that she will continue her course of medicines although symptoms may have subsided.

You can ask her to follow tips below to avoid any complications

1. Use sterile water to clean vaginal area (method is described by me and you can avoid buying and expenditure towards this).

2. Drink as much as citrus water (no salt no sugar).

3. Empty bladder as frequent as she can.

4. Eat healthy food, very freshly cooked (not even morning cooked stuff for dinner).

5. Keep vaginal area dry and wear only soothing cotton inner wears and even saries if needed.

Hopefully she will not get in this situation again. You are a wonderful husband.

Dear Archie,
Could just tell me whether infection relation with husband. Obiviously I don't have any symptom of infection. But my friends told me that sometimes it is better to check myself due to her recurrent infection when she was with me. I don't know whether it will give any benefit as she is in India now.

Dear Archie,
She went to her doctor today, and doctor has given Sporidex af 375 and Citralka liquid. I think Sporidex belong to the same category of Kefloxin, isn't it?. What is Citralka Liquid. Doctor also adviced to have Barley water. What we will do. Only go through the course and advice.
I think she has taken the matter very seriously drinking more than 3 litre per day.

Dear Sadique,

It's fair enough to get worried as you are. At the same time do not ignore treating infection is much more important than risking new life in her womb.

There are series of antibiotics exist. A few of them are absolutely safe during pregnancy. Urispas is FLAVOXATA HCl is used widely with precaution. I believe your Dr. knows her system well and decided to use this one.

I do not however know what is Cefloxin contents and won't be in a position to comment much.

Make sure your wife don't get in this situation after treatment. Ask her to drink plenty of liquids, fruit juices with Vitamin C are wonderful and empty her bladder as often as she can. If you are sexually active then try to use Condom, it's better to keep sterile environment than contaminating her sensitive area.

Your baby is going to be fine.

Dear Sadique,
I believe this is a big emotional turmoil you are going through. Yes, her Dr. prescribed first generation derivatives of Cephalosporin (brand name sporidex). She may have suspected some Gram positive infection, such as Streptococcus.

She will be OK hopefully. Drinking liquid is wonderful. Citralka is nothing but Disodium Hydrogen Citrate, Dr. may want to make sure that amniotic fluid is maintained and also nutrients essential to recover infection.

About your check ups, if you want you can go ahead. It's certain that you may have some infection yet not symptoms (asymptomatic infection) and if you like you can treat or you may not have anything. If your wife is going through Streptococcus then it's common to have this bacterium in vaginal area. If she had no UTI then before delivery Drs. usually want to make sure that vaginal path does not have Strep and they perform analysis and choose whether or not to treat patient before delievering baby.

Relax and take things positively (you and your spouse). It will be OK. I am glad she is drinking plenty of liquids. Ask her to not to clean vaginal area with regular water. Better if she boil water for 15 minutes and allow that to cool down and use this water to clean vaginal area (this will be sterile water as long as it was boiled and covered before use). She could even use the same water to mix with citrus fruit and drink it all the time. May sound tasteless but it surely will be free from any infectious agents.

Hope I have provided you with basic essentials to help your wife.

Dear Sadique,

Let me explain, she must not fear for antibiotics. If it's necessity she has to take them. Better to take antibiotic (some are safe) and she will have no troubles with her baby. But if she avoids visiting Dr. and infection get severe, this time antibiotic used will be of no use, infectious agents will be resistant to the dose. This will pose severe threat and in my opinion visiting Dr. is the best.

True that cranberry juice play miraculous role. She can continue taking citrus juice as much as she can. Bascially cranberry juice will help enhancing acidity in vaginal environment and bacteria are usually sensitivie to that.

Ask her to drinkg citrus juice (home made lemonade -avoid adding sugar and salt just squeeze citrus fruit) and not plain water. She should drink at least 2-2.5 liters of that each day.

One thing is true, Sadique neither you nor me will make difference. The person who will make difference is Dr. and she must visit him/her.

Bacterial infections can be dangerous while pregnant. She should not delay.

I am sorry could not provide words that you or your wife may wanted to hear.

I fully agree that there are restricted treatments while pregnant but it's absolutely possible to treat pregnant women with antibiotics. They do not cross placenta and or pose any threats to baby.

dear Archie,
She completed her medicines but still she got severe pain around pelvic area. she told that she drinks lot of water, citrus juice and tender coconut milk daily but no improveent on pain. now days she is not using panties also.
Also she has not gone to doctor yet due to the fear to take further antibiotics.
Archie, there are lot of websites that praises miracle effect of Cranberry Juice against UTI. is it true. My wife told me that Cranberry is not available in Kerala.
I don't know what will do as she is far away from me. I can only advice through telephone and letters.

Anyway I really appreciate your kind advice

Dear Sadique,

Yes the Kefloxin is brand name for Oral Cephalosporin, a kind of narrow spectrum antibiotic that specifically targeted to Gram positive pathogens of Respiratory tract, Urinary tract and renal functions. This antibiotic can be effective with certain Gram negative bacteria as well, the dose have to be determined from antibiotic sensitivity results of specimen culture.

I believe your Dr. has considered pros and cons while choosing. This antibiotic has not proven to casue any bad impacts on unborn child, may mean safe -mainy because there are no evidences showing bad effects. For the moment you need to be relaxed as it's going to be OK.
Preventive measures:
She has to maintain her personal hygine as high as she can. She already has had two round of infection. These infection can recur if not completely treated. Subside of symptoms does not mean bacteria were killed 100% only means the amounts of bacteria were far low to cause any symptoms. The best would be to follow full dose.

She should avoid any tight cloths around vagina and use loose fully cotton textured cloths. If vaginal discharge bothers then she can have cotton liner than synthetic.

She must maintain the vaginal area as dry as she can and someone must help her cleaning it every time of bath. It's going to be hard to clean by herself this zone as she is very pregnant and may not see it properly.

Drinking plenty of liquid and emptying bladder each time is highly recommended. This will dilute all ingredients and any causative agent from the bladder.

thanks a lot. she was in Qatar with me until last week. She went to India for Delivery. Before pregnancy she had infection and treated with antibiotics. Now same infection started, doctor in India did culture and prescribed Kefloxin500(I think Cefadroxil) and Urispas. Now she told me that pain on pelvic area has reduced. But I don't know whare are the precaution she has to take to prevent further infection.
Is Kefloxin a safe antibiotic?.
